Thank you for your interest in CHCS Service CareForce Network. As a member of this
team, you will be asked to perform onsite geriatric insurance assessments in your
community, providing the LTC insurers with detailed information for their benefit
and underwriting departments.
Basic requirements for the Assessment Specialists are:
- Registered Nurse with an active license in the State of residency.
- Minimum 5 Years of Nursing Experience
- Strong Observational Skills
- Geriatric or Home Health Care Experience A Plus

Assessment Specialists are utilized to perform the following
basic types of assessments:
► Benefit Determination Assessment:
To determine if the policyholder meets the criteria
of their policy and ensures that the appropriate level of care is in place.
► Underwriting Assessment:
Provides the Underwriter with a detailed look at an
individual applying for a Long Term Care Policy.
► Wellness Assessment:
The purpose of this assessment is care management with
the focus on health and wellness.
► Folstein Only Assessment:
This assessment helps the case manager determine the
cognitive status of the policy holder.
► Care Giver Assessment:
This assessment helps the case manager determine the
caregiver’s ability to meet the insured’s ADL and/or cognitive needs.
